Unban/unmute request form

You want to request an unban/unmute?
Then create a new post using the following form.

Request form:
Topic: Your Nickname (Nickname of the admin who banned/muted you)

1. Your Nickname
2. Your STEAM_ID
3. Link to your ban/mute
4. Link to your player information
5. Link to your demo
6. State the reason why you should be unbanned/unmuted. ( Writing "I'm not a cheater" is not enough)
